We kindly ask that you confirm your interest in ordering the alternative vaccine so that we can obtain a clear estimate of the quantity to be ordered. "The Government has made available Covid-19 vaccines to Thai citizens at no cost" Due to the high number of patients expressing interest in the alternative Covid-19 vaccine (Moderna), which is expected to arrive in October 2021, we kindly ask that you fill in this survey to confirm your intention of receiving the alternative Covid-19 vaccine (Moderna) and process your payment for 3,300.- baht (inclusive of 2 shots) ., in order for us to get a precise estimate to order (vaccination will be administered in order of payment). *The price above does not include a medical certificate and other fees not pertaining to the vaccination.

When available, the vaccination will be administered in the order of payment received. Scheduling of vaccination date and time will be notified in advance. 2.As Vibhavadi Hospital is not the direct sourcer of the vaccine, should there be a shortfall of supply or delay from the expected time of arrival as stated on page 1, you are eligible to choose the following: 2.1 Request a full refund, or 2.2 Wait for the next allotment Should the above unforeseen circumstance occurs, we will contact you in due course. 3. In the case where the alternative vaccination is made available within the period stated on page 1, no cancellations are allowed. Vibhavadi Hospital reserves the right to not give refunds, but will allow transfer of vaccination to other persons with one day advanced notice. 4. The main objective of the Covid-19 vaccine is to prevent the severity of the disease rather than to prevent contraction of the Covid-19 disease. Remark :

1.กรณีวัคซีนเข้ามาถึงโรงพยาบาลแล้ว ลำดับการฉีดวัคซีน จะเป็นไปตามลำดับการชำระเงิน โดยโรงพยาบาล จะทำการนัดหมาย วัน เวลา ให้ท่านทราบล่วงหน้า 2. โรงพยาบาลเปิดจองแพ็คเกจ 2 เข็มต่อท่านเท่านั้น โดยท่านจะต้องรับผิดชอบในการหาผู้ฉีดร่วม กรณีต้องการฉีดกระตุ้นเพียงเข็มเดียว 3.เนื่องจากโรงพยาบาล มิได้เป็นผู้จัดหาวัคซีนโดยตรง กรณีที่โรงพยาบาลได้รับจัดสรรวัคซีน ไม่เพียงพอต่อจำนวนสั่งจอง หรือล่าช้ากว่ากำหนด โรงพยาบาล เสนอให้ท่านเลือก ดังต่อไปนี้ 3.1 ขอรับเงินคืนเต็มจำนวน (กรณีหักผ่านบัตรเครดิตจะมีการหักค่าธรรมเนียม) 3.2 รอวัคซีนที่โรงพยาบาลจะได้รับจัดสรรใน Lot ต่อไป ซึ่งหากเกิดกรณีดังกล่าว โรงพยาบาลจะทำการติดต่อท่านที่ไม่ได้รับวัคซีนที่สั่งจอง ต่อไป 4.กรณีที่โรงพยาบาลได้จัดเตรียมวัคซีนให้ท่านแล้ว แต่ท่านแจ้งยกเลิก หรือแจ้งไม่ประสงค์ฉีดวัคซีน ไม่ว่าด้วยเหตุใด โรงพยาบาลขอสงวนสิทธิ์ที่จะไม่คืนเงิน โดยท่านสามารถเปลี่ยนสิทธิ์การฉีดวัคซีนให้ผู้อื่นได้ โดยแจ้งให้โรงพยาบาลทราบล่วงหน้า อย่างน้อย 1 วัน 5.เป้าหมายใหญ่ ของการฉีดวัคซีน โควิด 19 คือการยับยั้งความรุนแรงของโรค ซึ่งจะช่วยลดอัตราการเสียชีวิตของผู้ป่วย ไม่ใช่เพื่อมิให้คนติดเชื้อโรค โควิด 19
